I.C.E. Card Contents
An I.C.E. Card has two parts: A form and a protective sleeve, which easily fits in your wallet.
The I.C.E. form offers room for you to list the following information:
Order I.C.E. Cards and Refills
- Personal information—including name, address, date of birth and location of advance directives
- Emergency contacts
- Physician information
- Insurance providers
- Primary medical conditions
- Current medications and supplements
- Allergies / other information
If you have a durable power of attorney for health care form or a living will, you can indicate where those documents are located.
The Benefits of I.C.E.
Your I.C.E. Card will inform emergency medical technicians in the North Valley community about treatment options to avoid on the way to the hospital. If you arrive at a John C. Lincoln hospital, we will know exactly who we should contact on your behalf.
If you're a parent, an I.C.E. Card can give you peace of mind in situations when a babysitter may not be familiar with your child’s health information.
How to Use I.C.E.
Fill out the simple card for each member of your family. Place the completed card in the I.C.E. sleeve and encourage each family member to keep it with him or her at all times.
Be sure to update your I.C.E. Card anytime your information changes. We will be happy to send you new forms.
